Ciprofloxacin [Ciloxan] is a solution for infusion (to be given via a drip), which is used to treat some specific types (gram-negative) of severe and/or life-threatening bacterial infections. It can be used in patients (adults) who have:

  • pneumonia
  • septicaemia (blood poisoning)
  • complicated skin and soft tissue infections
  • osteomyelitis (bone infection)
  • intra-abdominal infections
  • complicated urinary tract infections

Children and Adolescents

Ciprofloxacin [Ciloxan] can also be used if you are 5-17 years old, have cystic fibrosis (CF) and have a respiratory tract (lung) infection caused by a bacteria called Pseudomonas aeruginosa. Cystic fibrosis (also called mucoviscidosis) is a hereditary disease of specific glands. It affects the lungs, sweat glands and the digestive system, causing chronic respiratory and digestive problems.

Ciprofloxacin [Ciloxan] should not be used:

  • if you are allergic (hypersensitive) to ciprofloxacin or chemically related substances (for example other fluoroquinolone antibiotics), or any of the other ingredients of Ciprofloxacin [Ciloxan] Solution for Infusion.
  • if you are pregnant or are breast-feeding, as the safety of the drug has not been tested in these patient groups.
  • if you are taking tizanidine (a drug used for spasticity).
  • if you have an existing tendon disorder (with pain and inflammation) caused by fluoroquinolone antibiotics.
  • in children and growing adolescents, except for those aged 5-17 years, who have cystic fibrosis and a respiratory tract (lung) infection caused by Pseudomonas aeruginosa bacteria.

Special care with ciprofloxacin will be taken:

  • if you are being treated for heart rhythm disturbances
  • if you suffer from epilepsy or if you have disturbances in the central nervous system or disturbances in cerebral (brain) blood circulation (for example increased susceptibility to fits,stroke).
  • when you spend time in the sun. Ciprofloxacin [Ciloxan] may cause an increased sensitivity to sunlight.
  • Avoid spending too much time in the sun and protect yourself carefully from ultraviolet radiation. Do not use sun beds.
  • if you develop severe and persistent diarrhoea during or after treatment: if this occurs, you should consult your doctor immediately.
  • if signs of tendon inflammation occurs (for example, as a painful swelling of the Achilles’ tendon): if this occurs, contact your doctor.
  • if you have a muscle disease called myasthenia gravis – taking ciprofloxacin may very rarely
  • make the symptoms of your disease worse.
  • to ensure that you receive sufficient fluids, to prevent a rare kidney problem where crystals form in the urine.
  • if you have a family history of, or have the hereditary condition Glucose-6-Phosphate Dehydrogenase deficiency.
  • if you are feeling depressed or confused after administration of ciprofloxacin. If this occurs,
  • you should contact your doctor immediately.
  • if you suffer an allergic reaction after receiving the first dose of this medicine.

If this happens, you should contact your doctor immediately. Signs of an allergic reaction could be: a sharp drop in blood pressure, paleness, restlessness, weak/rapid pulse, clammy skin, dizziness. In very rare cases these allergic reactions may lead to life-threatening shock.

if you get a skin reaction near the site that your infusion was given. These reactions may occur particularly when the infusion time is 30 minutes or less. They may take the form of reddening of the skin, irritation or pain, which usually disappears quickly after the infusion has stopped. If these reactions happen again or get worse when your next dose is given, no further infusions should be administered.

if you have a test for tuberculosis (TB) infection. Please tell your doctor if you have this test while you are receiving ciprofloxacin, as the results of the test could be affected by your medicine.

Using other medicines

Please tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including medicines obtained without a prescription. Tell your physician if you are using or have recently used any of the drugs listed below.

The blood levels of the following drugs may be increased when used with ciprofloxacin. Blood levels may need to be monitored and your dose adjusted.

  • theophylline (used to treat asthma and long-term breathing difficulties)
  • clozapine (used to treat schizophrenia)
  • tacrine (used to treat the symptoms of Alzheimer’s disease)
  • ropinirole (used to treat Parkinson’s disease)
  • tizanidine (used for relieving spasticity associated with multiple sclerosis or injury or diseases of the spinal cord)
  • mexiletine (used to treat abnormal heart rhythms)
  • glibenclamide (used to control diabetes) – the risk of hypoglycaemia may be increased
  • warfarin (used to ‘thin the blood’)
  • methotrexate (used to treat rheumatoid arthritis, psoriasis and, in large doses, cancer)
  • benzodiazepines (e.g. diazepam or midazolam, used as sedatives)
  • ciclosporin (used to prevent transplant rejection and conditions caused by inappropriate immune system responses)

The following drug may increase the blood level of ciprofloxacin if taken together:

  • probenecid (used to treat gout).
  • If you are taking phenytoin (used to control epilepsy), the blood levels may be either increased or decreased, so phenytoin levels should be monitored.

The following drugs taken with ciprofloxacin may have increased side-effects:

  • some non-steroidal anti-inflammatory drugs (NSAIDS) but not acetylsalicylic acid (aspirin) -used to relieve pain and inflammation

The following drugs should not be used with ciprofloxacin due to reduced levels of ciprofloxacin:

  • Opiate or anticholinergic premedication (prior to a general anaesthetic).

Pregnancy and breast-feeding

Ask your doctor or pharmacist for advice before taking any medicine.

Ciprofloxacin [Ciloxan] must not be used during pregnancy, as there is insufficient information regarding the effects of this medicine during pregnancy. If you suspect that you have become pregnant during treatment with ciprofloxacin, contact your doctor.

This medicine must not be used if you are breast-feeding, as ciprofloxacin passes into human breast milk. Breast-feeding whilst taking ciprofloxacin could harm your child.

Driving and using machines

Ciprofloxacin [Ciloxan] may affect your ability to drive and use machines. This effect is particularly noticeable in combination with alcohol.

Important information about some of the ingredients of Ciprofloxacin [Ciloxan] Solution for Infusion

Contains 9 g glucose in 200 ml solution for infusion. This should be taken into account in patients with diabetes mellitus.

How Ciprofloxacin [Ciloxan] is given

Dosage

The dose used will be decided by your doctor, who will adjust it especially for you. The dose depends upon the type, severity and site of infection and your general state of health (e.g. if you have kidney problems).

The usual dose in adults is 200-400 mg ciprofloxacin twice a day.

In the case of very severe infections, the dose can be increased up to a maximum daily dose of

1200 mg (400 mg three times a day).

Children and adolescents

For children and adolescents aged 5-17 years, who have cystic fibrosis and a lung infection caused by Pseudomonas aeruginosa bacteria, the usual dose is 10 mg ciprofloxacin per kg body weight. This dose will be given three times a day (maximum 1200 mg daily).

Dosage adjustment

If you are over 65 years of age, your doctor may prescribe you a different dose, which will be based on your kidney function and how serious your infection is.

If you have kidney problems, you should inform your doctor. He/she may need to adjust your dose if your kidneys are not working properly.

Method and route of administration

Ciprofloxacin [Ciloxan] is given intravenously (into a vein) by infusion over 60 minutes and will be administered to you by your doctor or nurse.

Duration of treatment

It is very important that you continue to receive ciprofloxacin for as long as your doctor prescribes it. Your doctor will decide how many days of treatment you need.

Possible side effects

Like all medicines, ciprofloxacin can cause side effects, although not everybody gets them.

Common side effects (occurs in more than 1 in 100 but less than 1 in 10patients): Feeling sick (nausea), dizziness, skin rash, diarrhoea, vomiting, stomach pains, headache, agitation, tremor. Increase in the number of some white blood cells. Inflammation at the site of injection, reactions at the site of injection. Transient elevation of the results of certain laboratory tests.

Less common side effects (occurs in more than 1 in 1000 but less than 1 in 100patients):

Tiredness, changes to the blood (for example anaemia, reduced number of white blood cells or platelets). Fast heartbeat, palpitations, heartburn (dyspepsia), flatulence (wind), loss of appetite. Transient elevations of certain liver enzymes, changes in prothrombin values (one of the substances that is needed in order for the blood to clot in a normal way). Pains and swelling in the joints. Hives, itchy skin. Sleeping difficulties, confusion. Shortness of breath, coughing up blood, muscle pain, thrush (Candida infection).

Rare side effects (occurs in more than 1 in 10,000 but less than 1 in 1000patients):

Hypersensitivity (allergy) reactions. Inflammation of the intestines (pseudomembranous colitis). Elevated blood sugar values. Anxiety, nightmares, depression, hallucinations. Increased or reduced sensitivity to touch, difficulties in the coordination of movements, epileptic fits. High or low blood pressure. Inflammation in the Achilles’ tendon, muscle pains, pain in the arms and legs, back pain, chest pain, fever. Crystals in the urine, blood in the urine. Swelling around the larynx (voice box), tingling in the hands and feet. Visual disturbances (for example double vision, colour vision disturbances). Inflammation of the pancreas.

Very rare side effects (occurs in less than 1 in 10,000patients): Anaemia caused by red blood cells breaking down. Increase in the number of platelets or white blood cells, bone marrow suppression. Impaired sense of taste and smell, migraine, fainting, symptoms of increased pressure in the brain, unsteady gait. Temporary hearing disturbances, ringing in the ears (tinnitus). Feeling of warmth (flush). Inflammation in the liver, or liver damage, which rarely leads to deadly liver failure, jaundice. Increased sensitivity to sunlight. Skin reactions (blisters, bleeding underneath the skin, swelling) and very serious skin reactions with damage to the skin and mucous membranes, sweating. Worsening of the symptoms of myasthenia gravis (muscle disease), cramps, inflammation of the tendon sheath. Inflammation of the kidneys, temporary reduced kidney function (including temporary kidney failure). Weakness. Increase of certain digestive enzymes. Irregular heart beat. Psychosis and self destructive behaviour.

If you develop severe and persistent diarrhoea, you should consult your doctor immediately.

Single cases of completely or partially ruptured Achilles’ tendon, have been reported, particularly in elderly patients that have been treated with cortisone medicines. If symptoms of inflammation of the Achilles’ tendon (for example painful swelling) occurs, contact your doctor immediately.

In case of quickly developing a rash, itching, difficulties in breathing, fever and/or swelling of the face, contact a hospital or your doctor and discontinue the treatment.

How to store Ciprofloxacin [Ciloxan]

Your medicine will be stored and administered by healthcare professionals, who will follow this guidance:

Keep out of the reach and sight of children.

Do not use the medicine after the expiry date which is stated on the label (after EXP:). The expiry date refers to the last day of that month.

Do not store above 25°C. Do not refrigerate or freeze.

Keep the infusion bag within the sealed outer foil wrap before use, in order to protect from light and evaporation.

For single use only.

Use immediately after first opening the infusion bag.

Medicines should not be disposed of via wastewater or household waste. Ask your pharmacist how to dispose of medicines no longer required. These measures will help to protect the environment.

The active substance is ciprofloxacin.

1 ml solution contains: 2 mg ciprofloxacin (as ciprofloxacin lactate).

The other ingredients are glucose monohydrate, lactic acid, hydrochloric acid and Water for Injections.

What Ciprofloxacin [Ciloxan] looks like and the contents of the pack

Ciprofloxacin [Ciloxan] is a solution for infusion. It is available in transparent infusion (PVC or polyolefin) bags that contain 50 ml, 100 ml or 200 ml sterile solution of ciprofloxacin 2 mg/ml. The bags are sealed in an outer packaging of a double sheet of metallised polyethylenterephthalate.

Pack sizes:

5 x 50 ml, 10 x 50 ml, 50 x 50 ml 5 x 100 ml, 10 x 100 ml, 50 x 100 ml 5 x 200 ml, 10 x 200 ml, 20 x 200 ml

Not all pack sizes may be marketed.

Ciprofloxacin [Ciloxan] Solution for Infusion should be administered without mixing with any other substances or infusion fluids. Ciprofloxacin [Ciloxan] infusion has been shown to be compatible with Ringer’s solution, 0.9% sodium chloride solution, 5% and 10% glucose solutions and 10% fructose solution, when infused in parallel. Unless compatibility is proven, the infusion solution should always be administered separately.

Incompatibilities

Ciprofloxacin [Ciloxan] is incompatible with injection solutions (e.g. penicillins, heparin solutions), which are chemically or physically unstable at its pH of 3.9 – 4.5.
